Job Description

Job Title: Community Liaison

Department: Home Health

Reports to: VP, Strategy & Innovation

Job Summary:

The Community Liaison is responsible for the referral process for new admissions and developing client and vendor relations that ensure a steady admission flow.

Duties/Responsibilities of the Community Liaison:

Responsibilities of Community Liaison include, but are not limited to, the following:

  • Conducts daily marketing activities including collaborating with the Executive Director, Clinical Manager, and Intake Coordinator to establish and implement marketing initiatives.
  • Successfully navigate facilities, hospitals, and doctor’s offices to identify appropriate services that meet the patient's needs.
  • Employs marketing and promotional initiatives to achieve census projections.
  • Establishes and maintains positive working relationships with current and potential referral and payer sources.
  • Builds and monitors community, customer, and patient perceptions of Affirmation Home Health as a high-quality provider of services.
  • Contributes to organizations’ strategic planning including identifying opportunities for additional or improved services to address patients' needs.
  • Collaborate with VP of Strategy and Innovation to maintain a comprehensive working knowledge of Affirmation Home Health markets including government agencies, major payer groups, key referral sources, and competitor’s market positioning.
  • Maintains comprehensive working knowledge in the field of marketing and shares information with appropriate organization personnel.
  • Documentation: Use Trella daily to document all communications between facilities, physicians, patients, and referral sources immediately after visits to preserve important details.
  • Support Medical Records with any orders that may require hand delivery & pick up from local physician offices and facilities.
  • Adhere to HIPAA and patient information privacy laws and regulations.


Required Skills/Abilities:

  • A bright, energetic professional with outstanding communication and interpersonal skills.
  • Entrepreneurial-minded with an internal drive to hit goals, succeed and continuously develop.
  • Proven record of successful performance, exceeding monthly quota objectives through outbound sales activities.
  • Proficient with corporate productivity and web presentation tools
  • Strong computer skills and experience working with a CRM.
  • Excellent verbal and written communication skills are matched with firm quantitative skills.
  • Excellent working knowledge of primary home health services and protocols.
  • Demonstrable working knowledge of Medicare and non-Medicare reimbursement requirements including eligibility for service, managed care, and Medicare/managed care outpatient therapy services requirements.


Education and Experience:

High school diploma or equivalent.

At least two years of related experience is required.


Physical Requirements:

Prolonged periods of sitting and standing.

Must be able to lift up to 15 pounds at times.
